How to Place an Order

It's Easy to Place an Order!

1. All new orders and reorders begin with a PDF Order Form.

2. Print the appropriate script/order form.

Note: Save the downoaded PDF form to your PC for future reorders.

3. Complete the order form with:

Contact information
Shipping Information
Product selection
Payment information.
Include signature as a licensed Dentist.

4. Send the form by:

Fax your order form to 1-888-984-2010.

#2 EMAIL

Scan and save the completed order form.

Email as an attachment to order@dentalpharm.com.

#3 MAIL / CHECK

Mail completed form. If paying by check, include check with form and mail to:
